Here's how it all goes down: once a driver signs in with a PayPal account, they can pull into a supported Cumberland Farms location and fire up the iOS/Android app or the mobile website. From there, the app uses the device's GPS to hone in on their gas station, though failing that users can also input the station number. After punching in their pump number, voila -- their gas charges are sent to PayPal, and users get an email receipt.

China punishes social media, websites on coup talk

(AP) ? China is closing a dozen websites, penalizing two popular social media sites and detaining six people for circulating rumors of a coup that rattled Beijing in the midst of its worst high-level political crisis in years.

The extensive clampdown, announced late Friday by state media, underscores the authoritarian government's anxieties over a public that is wired to the Internet and eager to discuss political events despite censorship and threats of punishment.

A Xinhua News Agency report said Beijing police questioned and admonished an unspecified number of Internet users and detained six people not further identified. Aside from the 16 websites shut down, two Twitter-like services run by Sina Corporation and Tencent Holdings, which each have more than 300 million users, said they would disable their comment functions for three days in a necessary "concerted cleanup."

Users of the services ? the most unfettered media in China ? received a notice when they tried to post comments explaining the suspension was due to the many "rumors and such illegal, harmful information." Original postings are still allowed, and some used them to castigate the government.

"Every time you murder free speech, we all take note of your reaction. Your gradual loss of public credibility is the result of such messes," Wang Gongquan, a private equity executive, wrote on his Sina microblog account, which has 1.3 million followers.

Both the coup rumors and the crackdown show how the firing two weeks ago of Bo Xilai, the populist leader of the mega-city of Chongqing, has brought leadership struggles out of the usually closed confines of elite Communist Party politics and into the public.

Yet to be fully explained, Bo's dismissal came after a top aide fled temporarily to a U.S. consulate, apparently to seek asylum and in violation of party rules. It also came as the senior leadership gears up for a handover of power to a younger generation leaders in the fall, always an a period of intense bargaining.

Politically minded Chinese saw the removal of Bo, considered a contender for a top job only months ago, as a sign of divisive infighting. Speculation about Bo's fate and that of others ricocheted across microblogs and spiraled into talk of troop movements and gunshots around the leadership's Zhongnanhai compound in central Beijing on March 19.

The Internet sites and people punished fabricated and disseminated rumors that "military vehicles are entering Beijing, something is going wrong in Beijing" and similar posts, Xinhua said, citing the State Internet Information Office, an interagency body charged with policing the Internet.

Over the past week, President Hu Jintao and other senior leaders have tried to project an image of unity and reassert control over the public message. Censors have furiously blocked and unblocked a range of terms to quell errant microblog chatter; "coup" remained a banned search term Saturday.

"Internet rumors and lies packaged as 'facts' will turn conjecture into 'reality,' stir up trouble online and disturb people's minds," the party's flagship newspaper People's Daily said in a commentary accompanying the announcement of the clampdown. "If allowed to run amok, they will seriously disrupt social order, affect social stability and harm social integrity."

The Twitter-like microblogs, with their huge user numbers and rapid-fire postings, have proved a challenge for Beijing and its Internet monitors. Social media played a role in forcing the government to disclose more information after a crash on the showcase high-speed rail network last summer and to provide more information on the thick air pollution in Beijing and other cities.

A new rule requiring social media users to register accounts under their real names and identity card numbers by the middle of March failed to deter the rumors over Bo or a coup.

Following the detentions, shutdowns and other penalties, Pan Shiyi, a celebrity real estate developer and prolific microblog poster with 9 million followers, questioned Saturday whether the tactics were "the right medicine" to deal with rumors. Pan had not directly posted about a coup but had noted the heavy-handed censorship. To dispel rumors about himself, he said: "I'm not among the six."

A scheme to help migrants forced to leave relationships as a result of domestic violence is being made permanent, the Home Office has said.

The move follows a pilot project that has provided support for 1,522 people, including 738 women with children.

The initiative assists victims who would otherwise be destitute or have no access to public funds.

Immigration Minister Damian Green said domestic violence affected people of all ages and backgrounds.

?Terrible crime?

The scheme helps spouses and partners, who are foreign nationals and the victims of domestic violence, with access to support services.

The government said the project would assist an estimated 500 people a year in the UK.

Immigration Minister Damian Green said: "Domestic violence is a terrible crime affecting people of all ages and backgrounds and this government is determined to tackle it.

"No one should be forced to stay in an abusive relationship and this scheme helps victims in genuine need escape violence and harm and seek the support they deserve."

The scheme will become permanent in April following a pilot called the Sojourner Project which has been running since 2009.

The government said that in many cases victims were afraid to seek help because they lacked financial support and feared they would be removed from the UK if their relationship failed.

?Saved lives?

Those eligible for the scheme will be granted a limited period of exceptional leave to remain by the UK Border Agency.

Victims would be able to access financial and support services, such as a refuge, and be allowed to apply for UK residency.

Jo Clarke, from refuge charity Eaves Women's Aid, who co-ordinated the pilot, said: "The Sojourner Project pilot has been a huge success, enabling in excess of 1,000 people, 12 of them men, to escape abusive relationships and secure indefinite leave to remain in the UK.

"Victims of domestic violence with no recourse to public funds are among the most vulnerable and badly abused so the Sojourner funding has, quite literally, saved many lives.

"Our findings have demonstrated the need for this escape route and I welcome the introduction of the government's long-term solution which will mainstream the provision of financial support to this group."

On the front of the tower you'll find a Blu-ray (BD-RE) optical drive, and a 65-in-1 card reader that accommodates every major media card format, including SD/MMC, microSD, CF/MD, MS/Pro/Duo, and xD, plus another USB 2.0 port thrown in for good measure. On the back of the tower you'll find scads of connections, with six USB 2.0 ports (one of which is occupied by a Bluetooth receiver), four USB 3.0 ports, a USB BIOS flashback port, and two USB/eSATA combination ports. The Gigabit Ethernet port is your only networking option, as the Silent Gaming PC has no Wi-Fi. Display outputs include two DVI, one HDMI, and one full-size DisplayPort. There are also several audio outputs, with ports for S/PDIF and eight-channel analog audio.

Pull off the case's side panel (the thumbscrews make it easy) and you'll find that the Quiet X79 Gaming PC is equipped with both a 120GB SSD (which serves as the OS drive) and a silent 2TB Western Digital hard drive (for storage). Plugged into the Asus P9X79 motherboard is a six-core Intel Core i7-3930K processor?the same model found in both the Chimera 4-V1 and the Velocity Micro Raptor Z90 (Intel Core i7-3930K) ($4,999 direct, 3.5 stars)?equipped with a giant heat sink?fan unit to keep it cool, and an overclocked Asus video card based on the Nvidia GeForce GTX 580 GPU. A quintet of whisper-quiet fans keep everything cool.

An 850-watt power supply provides more than enough juice for all of the Quiet X79 Gaming PC's components, and additional hardware if you choose to upgrade?and there are plenty of upgrade options available. Though the system comes equipped with 16GB of RAM, there are four open DIMM slots on the motherboard, which supports up to 64GB total RAM. You'll find four open PCI Express (PCIe) slots, though the two PCIe x1 slots are blocked by the bulky video card, and one open 5.25-inch drive bay; four SATA ports are also free.

Because this is a custom gaming rig, you'll find that the hard drive is immaculate, with the only software installed being Windows 7 Professional. The Quiet X79 Gaming PC also comes with AVADirect's Silver Warranty Package, a three-year warranty on parts and labor.

Performance
AVADirect Quiet X79 Gaming PC In our tests the Quiet X79 Gaming PC proved it's able to mix it up with much more expensive gaming rigs. It tore through our processor-intensive Photoshop CS5 test in 2 minutes 31 seconds?ahead of the Polywell Ignition X7900i-3960 (2:34) and the iBuypower Chimera 4-V1 (2:42), but behind the 2:21 of the Digital Storm ODE Level 3 ($2,399 direct, 4.5 stars) and the 2:22 of the Velocity Micro Raptor Z90 (2:22). It finished Handbrake in 58 seconds, beating out the Chimera 4-V1 (1:03) and tying with the Ignition X7900i-3960 (0:58), but falling short of the ODE Level 3 (0:56).

AVADirect Quiet X79 Gaming PC

The Quiet X79 Gaming PC pumped out respectable frame rates in our gaming tests, cranking through Crysis and Lost Planet 2 at 43 frames per second (fps), both at 1,920-by-1080 resolution with 4x anti-aliasing and high detail settings. Although these are playable scores, they can't match those of rigs running multiple graphics cards, like the ODE Level 3 (71 fps in Crysis, 122 fps in Lost Planet 2), which is equipped with two GTX 570 cards, or the Velocity Micro Raptor Z90 (44fps in Crysis, 81fps in Lost Planet 2), which contains two GTX 580 cards.

The AVADirect Quiet X79 Gaming PC offers impressive performance and graphics for its relatively modest price tag. Honohan Says Ireland Can't Afford Home-Loan Debt Relief | Capital ...

DUBLIN ? The Irish government has pumped enough money into its broken banks, but the bailed-out country cant afford to provide major relief of home-loan debts, and will need to carefully tailor plans to help distressed borrowers, the head of the Irish central bank said Wednesday.

For its part, the Irish central bank will review its so-called mortgage arrears code and allow banks to be more actively involved with debtors, Patrick Honohan told a gathering of lawyers in Limerick City.

Ireland slid into a European Union and International Monetary Fund bailout program in November 2010, when its banks could not absorb huge losses on property loans when the countrys property bubble burst.

However, the government does not have the money to help all distressed home-loan borrowers and must direct fair and technically efficient aid to those most in need, Honohan said.

Although the banks have been provided with enough capital to allow for loan modifications in respect of loans that are clearly unaffordable, unnecessary debt forgiveness could quickly erode that buffer, placing a further burden on the government finances, which they are in no condition to absorb, he said.

The government has said it will implement recommendations of its Interdepartmental Group on Mortgage Arrears, known as the Keane report, that was published late last year, and bring in new plans announced in January for new insolvency and bankruptcy laws.

Irish central bank figures published last month showed the number of Irish residential home loans in arrears had increased again, with 9.2% of all mortgage home accounts in the Irish republic in arrears for more than 90 days at the end of December.

And approximately 14% of all accounts were in arrears for over 90 days or had been restructured and continue to perform, the central bank figures showed.

However, Honohan said Wednesday that most home-loan borrowers whose properties are worth less than their mortgage debt are servicing their loans.

Retail Business & Technology Expo, New Spring Leaves ...

Last week I attended the Retail Business and Technology Expo in London. It?s the second year of the show. Their strategy to drive traffic appears to be paying off ? registered attendees were reportedly up significantly.? Floor space was at a premium (vendors that typically buy whole pads at NRF in January had almost miniscule booths by comparison ? in a capacity venue).

There was one particular thing that caught my attention and inspired me to make the long haul from Denver to London for a two-day conference. It was the combination of ?business? and ?technology? in the conference?s title. In an age when technology increasingly lies at the heart of almost everything that retailers want to accomplish in engaging consumers, it was refreshing to see a conference focus on bringing the two together.

So what did I learn from this event? One, it pulls a lot of Americans, much like NRF?s big show manages to pull a lot of Europeans. I saw a lot of familiar faces at the event ? established US players alongside companies looking to break out into the European market by establishing a toe-hold at the London show.

But it also drove home to me that ?omni-channel? as a term is here to stay. It was whole-heartedly embraced by attendees and speakers alike, with a fairly common definition: the stuff that comes after you realize that just enabling simple, one-off process that help customers cross channels isn?t enough. And the retailers who spoke at the conference graciously shared their experiences as they struggle with building cross-channel processes.

In very general terms, European retailers seem to have more challenges than American retailers, in part because they have a more established version of a channel that is very immature in the US ? home delivery. Home delivery is not a universal capability, by any means, but it does serve as more of a consideration for omni-channel in European cities than it does in most US cities, outside of New York, and maybe Boston and Chicago. It doesn?t mean that European retailers have bigger challenges, or even significantly different challenges, when it comes to enabling cross-channel shopping behavior, but it does mean their challenges are maybe slightly more complex because they have more touchpoints for consumers.

However, one thing that also came across loud and clear was that customer centricity remains at the heart of cross-channel. All of these efforts that retailers are struggling to align to a cross-channel reality come at the insistence of shoppers, who persist in crashing across channels with no regard to retailers? constraints in supporting cross-channel behavior. So whether it is a grudging, after-the-fact acknowledgement of the customer as central to the business, or an enthusiastic embrace of same, retailers ranging from McDonald?s to Tommy Hilfiger demonstrated at the conference that customer centricity is key to unlocking omni-channel value ? when you place customer considerations seriously at the heart of your strategy, you can create truly innovative solutions to customer problems that you maybe didn?t even know existed. And that is a straight path to differentiation ? and long-term customer value.

So I hope the show continues its success, and grows into a real intersection point between technology and the business ? reflected not only in the discussions and presentations but also in the cross-section of attendees. A technologist who can speak business is valuable. But increasingly, a business owner who can speak technology is meeting a basic job requirement. So many people only associate health with the state of their physical bodies. Maybe this is you? Improving outcomes in gynaecological cancer: the benefits of ...

Over the past 30 years advances in medicine have resulted in increasingly complex care pathways and individualisation of care. As a result, medicine has become subspecialised in many areas, and there is evidence for improved outcomes for patients treated by multidisciplinary teams, as demonstrated in chronic heart failure,[1] stroke,[2] and colorectal cancer.[3]

The 1995 Calman-Hine report, which aimed to improve cancer survival in the UK, recommended setting up co-ordinated care within cancer networks;[4] patients with less common and rare cancers should have care centralised, with patients referred from units to a centre, which could devote a specialist multidisciplinary team to the care of a particular tumour site.

Gynaecological malignancy, arising from the ovary, cervix, endometrium, vulva or vagina, affects 2.2% of the female population by the age of 65 and is the second most common cause of cancer death in women, following breast cancer.[5] In the developed world, most gynaecological cancers are endometrial or ovarian, whereas cervical cancer is more common in the developing world and is the leading cause of gynaecological cancer death worldwide. However, compared with colorectal and breast cancer, gynaecological cancer is relatively uncommon.

Treatment for gynaecological cancers is frequently multimodal, requiring co-ordination of surgical care with chemotherapy and radiotherapy. For some time there has been indirect evidence that women have more favourable outcomes if they are treated by specialist gynaecological oncologists in cancer centres. Gynaecological cancers were one of the first cancer sites to have centralisation of care recommended following the Calman-Hine report, as detailed in the Improving Outcomes in Gynaecological Cancer report in 1999.[6] Furthermore, gynaecological oncology has become a subspecialty within obstetrics and gynaecology in many countries, with the development of specific advanced training programmes to meet the technical challenges of gynaecological cancer surgery and to understand the role of surgery within a multidisciplinary team.

A Cochrane Review by Yin Ling Woo and colleagues, published in the March 2012 issue of The Cochrane Library, evaluates the effect of centralisation of care for women with gynaecological malignancy.[7] Outcomes for women with gynaecological cancer treated by specialist gynaecological oncologists within centres were compared with those receiving care in non-specialist general hospitals. The review identified five studies, including 62,987 women with gynaecological cancer, and concluded that women with gynaecological cancer may have improved outcomes if treated in specialist centres. The findings were stronger for women with ovarian cancer than for the other gynaecological cancers, as several of the studies examined ovarian cancer only. A meta-analysis of data from three of the studies (including over 9000 women) demonstrated that women with ovarian cancer who received care in hospitals with a gynaecological oncologist on site had improved survival compared with those treated in non-specialist hospitals; hazard ratio (HR) of death was 0.90 (95% confidence interval (CI) 0.82 to 0.99).

The review authors noted that all the included studies were from high-income countries (USA, Canada, UK, and the Netherlands), so the findings may not be transferable to other healthcare settings. The review was also limited by the poor quality of the evidence: all included studies were retrospective observational studies and therefore at high risk of bias. However, the studies demonstrated consistent results, which gives some weight to the findings. Another limitation was that only one study, albeit of 48,981 women, included women with gynaecological cancer other than ovarian, and the authors suggested that further studies, ideally from other healthcare settings, should be performed to confirm the benefits of centralising care for women with other gynaecological cancers. The authors thought that the likelihood of selective reporting bias was low, as all studies included overall survival data. However, none of the studies looked at risk of harms or quality of life data, which would be important to women and those commissioning healthcare services.

Gynaecological cancer is another disease for which specialisation seems to improve survival outcomes for patients. Why should this be? Seeking reasons for the benefit of subspecialised care was beyond the scope of the Cochrane Review, but one can speculate on factors that may play a role. Gynaecological cancer surgery can be challenging, involving techniques not routinely encountered by generalists. Even in early-stage ovarian cancer, surgery by a specialist gynaecological oncologist is an important prognostic indicator.[8] For advanced ovarian cancer, several studies have demonstrated that women operated on by a subspecialty trained gynaecological oncologist are more likely to be optimally debulked than women operated on by a general gynaecologist. Interestingly, one study demonstrated that centralisation resulted in a two-fold increase in optimal debulking rates for women with stage III-IV ovarian cancer, but there was no significant change in survival, suggesting that survival in advanced ovarian cancer is influenced primarily by factors other than surgical expertise.[9]

In vulval cancer, a condition for which optimal surgery has a major influence on survival, access to subspecialist surgery due to implementation of Calman-Hine guidelines has improved lymphadenectomy rates and survival.[10] So surgical skill clearly has an effect, and access to appropriately trained multidisciplinary care is important, as demonstrated by Woo and co-authors.[7] Another factor may be that women who are very unwell, with advanced disease and poor performance status, may not be fit for transfer to a specialist centre, although some studies have demonstrated that women treated at specialist centres have more advanced disease.[11]

The authors concluded that it would be important for future studies to have a more robust prospective design, although recognised that a randomised controlled trial may be difficult, but that prospective studies should be performed, with adequate funding and agreed protocols, to evaluate the impact of instigating centralisation of care in the future. Certainly any future studies should ideally examine other gynaecological cancers and outcomes, such as risk of adverse outcomes, quality of life, and cost-benefit analyses, in addition to survival, to inform future healthcare commissioning.

The evidence in gynaecological cancer treatment is consistent in demonstrating benefits from centralisation and is in line with evidence from other diseases (and mirroring results from a new Cochrane Review of colorectal cancer surgery[12]), thus supporting the role of cancer networks and the need for co-ordination of care. These improvements need to be safeguarded in these difficult economic times, and co-operative working between centres and units needs to be protected from the effects of competition, for the benefit of our patients.

Gynaecological?cancers are cancers affecting the ovaries, uterus, cervix, vulva, and vagina.? They are the second most common cancers among women, after breast cancer. It is often suggested that outcomes are improved by centralising care within highly specialised services that include expert surgeons, radiologists, pathologists, oncologists who specialise in chemotherapy and radiotherapy, specialist nurses and other health professionals. However, consensus is lacking on whether centralisation of care for gynaecological cancer helps patients to live longer. This review investigated this issue by comparing the survival of women diagnosed with gynaecological cancer who received care from specialised and unspecialised centres.

We used a set of tests to ensure that the evidence the five studies identified reached the quality standard for our analysis.The analysis of three studies combined (meta-analysis), assessing over 9000 women, suggested that institutions with gynaecologic oncologists (specialists in the field of gynaecological cancer treatment) on site may prolong the lives of women with ovarian cancer compared to community or general hospitals. Similarly, another meta-analysis of three studies which assessed well over 50,000 women, found evidence to suggest that teaching centres or regional cancer centres (specialised centres) may prolong the lives of women with gynaecological cancer compared to community or general hospitals. The largest study in this meta-analysis assessed all gynaecological cancers in 48,981 women, so it had major influence on the final result; this means that our findings are likely to be relevant to other gynaecological cancers, besides ovarian cancer.

Overall, the findings suggest that centralisation of care may prolong the lives of women with gynaecological cancer, and in particular ovarian cancer. However, the results should be interpreted with caution as all of the studies included in the review could be biased. For example, it is possible that the patients who were treated in specialised centres were less ill to begin with. Another weakness of the review is that only one of the studies included women with gynaecological cancers other than ovarian cancer.

Ideally, further studies in this area are needed.? New studies should be designed to avoid the possibility of bias due to the treatment of women at specialist and non-specialist centres being systematically different. Additionally, studies should assess the impact of centralisation of care on the quality of life of patients.

Most of the available evidence was about ovarian cancer in developed countries;?future studies should be extended to other gynaecological cancers and to less developed countries.

Copenhagen, 16 March 2012: Poor dental hygiene behaviours in patients with congenital heart disease are increasing their risk of endocarditis. Teens with congenital heart disease floss, brush and visit the dentist less than their peers. But they have healthier behaviours when it comes to alcohol, cigarettes and illicit drugs. Adults with single ventricle physiology (a type of congenital heart disease) also have poorer dental hygiene practices than their peers despite having better health behaviours overall.

The findings were presented in two studies at the 12th Annual Spring Meeting on Cardiovascular Nursing, 16-17 March, in Copenhagen, Denmark.

"Patients with congenital heart disease are diagnosed and receive their initial treatment in childhood but this does not mean that they are cured," says the supervisor of both studies, Professor Philip Moons, professor in nursing science at the University of Leuven, Belgium, and guest professor at Copenhagen University Hospital, Denmark. "They remain vulnerable for developing complications for instance we know that in patients with congenital heart disease, binge drinking can trigger life-threatening arrhythmias and good dental hygiene helps prevent endocarditis."

For the first study (FPN 34) 1, lifestyle information was collected from 429 adolescents with congenital heart disease aged 14-19 years from the longitudinal study i-DETACH (Information technology Devices and Education programme for Transitioning Adolescents with Congenital Heart disease). Of these, 401 were matched with a control of the same age and gender without congenital heart disease. All participants completed a questionnaire, developed by the research group of Professor Moons, which measures the use of alcohol, cigarettes and illicit drugs, dental care and physical activity. These behaviours are particularly important to the health of patients with congenital heart disease.

Using results from the questionnaire, the researchers calculated risk scores for 'substance use' (binge drinking; smoking; illicit drug use) and 'dental hygiene' (no dental visits; not brushing; not flossing) ranging from 0?. An 'overall health risk score' (range 0?) was calculated using the substance use risk score, dental hygiene risk score, and the absence of physical activity. The 3 risk scores were transformed to a scale ranging from 0 (no risk) to 100 (maximum risk). Scores were compared across different age groups.

The results reveal that health risk behaviours are prevalent in adolescents with congenital heart disease and they increase with age. They also show that in general, the health behaviour of adolescents with congenital heart disease is better than their peers except for dental hygiene.

Professor Moons says: "The fact that adolescents with congenital heart disease have better health behaviour overall than the general population is understandable given the amount of input they have had from healthcare professionals over their lives. But we need to do more to understand why their dental hygiene is not as good as expected."

For the second study (FPN 158), the same questionnaire was used to collect lifestyle information from adults aged 16-48 years (average age 24 years) with a type of congenital heart disease called single ventricle physiology. "This is a very complex congenital heart condition and we know that these patients are more at risk for endocarditis and arrhythmias than the larger population of congenital heart disease patients," says Professor Moons. "This means that their health behaviour is even more important."

A cross sectional, case control study was conducted in 59 patients who were matched on age and gender to 172 healthy controls.

In patients with single ventricle physiology, 85% drank alcohol; 26% were binge drinkers; 20% smoked cigarettes; 12% used cannabis over the past year; 20% had not visited the dentist during the last year; 46% were not flossing teeth; and 39% were not physically active.

Compared to healthy controls, patients with single ventricle physiology had better health behaviours overall. But patients exercised less and their dental hygiene practices (mainly flossing) was poorer.

The only statistically significant differences between patients and controls were for binge drinking and physical activity. A significantly lower proportion of binge drinking was found in patients compared to controls (24% vs 41%). And patients exercised less than controls (61% vs 76%).

"Patients with complex conditions can have physical restrictions so the fact that they are less physically active is perhaps not surprising," says Professor Moons. "But the reasons behind the poorer dental hygiene practices of patients in both studies need to be investigated further."

In the past, efforts to prevent endocarditis in patients with congenital heart disease focused on taking antibiotics one hour before a dental procedure. This prevented bacteria released into the blood from damaged gums travelling to the heart and causing an infection (endocarditis).

But researchers have since discovered that daily dental hygiene is more important for preventing endocarditis than antibiotics before a procedure. Professor Moons says: "This was a change in the American Heart Association 2007 guidelines but not all physicians have switched to that new paradigm. And of course if you switch to that paradigm you also need to educate patients in a systematic way, but a lot of centres are not doing that yet."

He adds: "Systematic structured patient education on the importance of dental hygiene is critical for preventing endocarditis in patients with congenital heart disease."
